### What is a Медична книжка?

The медична книжка (książeczka zdrowia) is an official medical документ issued by Охорона здоров'я установи in Poland. It serves as a personal medical record that travels with you throughout your life.

### Where to Get It

**For newborns and діти:**

### How to Obtain a Медична книжка
